Location New York, New York Shift: Day (United States of America) Description: Social Worker - OB Cohen Hospital - F/T Days At NewYork-Presbyterian, Weill Cornell social work is a life-changing career journey focused on amazing patient outcomes every step of the way. Together, we improve the health of patients and their families, making our communities - and the world - stronger The social worker is an integral part of the Interdisciplinary team supporting patients through their hospital course to a seamless and safe discharge. The social worker specializes in working with families providing comprehensive psychosocial, mental health, substance use and safety assessments, coordinating discharge planning for this vulnerable patient population. The social worker provides supportive counseling, linkage to community agencies and support, and referrals to homecare agencies to support a safe transition from the hospital. This is a full-time day shift, position with rotating weekend and holiday coverage. Preferred Criteria:
